First, the basics: On the ketogenic diet, at least 70 percent of your daily calories come from fat. Five to 10 percent of your calories come from carbohydrates (20 to 50 grams a day). The rest, up to 25 percent of your daily energy, comes from protein. By contrast, the healthy diet recommended by the Institute of Medicine is 45 to 65 percent carbs, 20 to 35 percent fat and 10 to 35 percent protein.

The ketogenic diet’s low-carb target can be met only by avoiding grains, dairy products, fruit, and legumes such as chickpeas and lentils. Starchy vegetables such as sweet potatoes and squash are out, and even amounts of lower-carb vegetables are limited.

So what’s left to eat? Typically, eggs cooked in butter for breakfast; for lunch and dinner, meat, chicken or fish with salad or green vegetables and plenty of oily dressing. I have an obvious interest in this subject because I am a man and I am middle-aged. Though, to be honest, I am technically only “middle aged” if I live till I’m nearly 120.

Like most people, I’m keen to stay as young and vigorous as possible for as long as possible. No male member of my family has lived longer than 74 years, a record I am keen to break.

Although I trained as a doctor, I have spent the last couple of decades making films for the BBC (programmes such as Trust Me I’m a Doctor and Horizon), during the course of which I’ve been able to pick the brains of health experts all around the world. As a result of what I’ve learnt, I have changed how I live. The following advice is not necessarily what you’ll hear from your doctor, but it is based on the latest science.

1 Measure up

Middle-aged men are prone to self-delusion, thinking they are slimmer, fitter, healthier and far more handsome than they really are. A reality check is useful, and if you keep a record of your measurements you can also tell if your new health regime is working.

A good place to start is with your heart rate, measured first thing in the morning.
Having a resting heart rate over 80 is a bad sign. A recent study of 11,000 people found that those with a heart rate above 70 beats per minute had significantly shorter lives than those who were noticeably below that figure. A top athlete will have a resting pulse of 40 or less. Mine is currently around 60. Losing weight and getting fitter will bring your pulse rate down. Six months into the program, they were randomly assigned either to use a website to monitor their diet and physical activity each day or to wear a fitness tracker that had a Web-based link to track activity and provide feedback for both exercise and diet. At this point, average weight loss was essentially the same in both groups. 

After 18 months of periodic measurements, improvements in diet, physical activity and body composition were comparable for the two groups. However, those who wore the fitness trackers lost less weight, on average, than the others. In the 24-month period, people wearing the trackers lost, on average, 7.7 pounds, while those who did not wear the devices dropped an average of 13 pounds. When you consider it takes up to 40 weeks or more to create a tiny human, you would think mothers would relax and give their bodies time to restore. Instead, according to a 2014 survey of 1,500 women in the United Kingdom, 40 per cent of new mums feel pressured into losing weight quickly. A 2015 study by the American journal Obstetrics & Gynecology found that 75 per cent of women don’t achieve their pre-baby weight one year post-birth, while UK-based research revealed it takes an average of 19 months to get close to pre-pregnancy weight.


"In my practice I see a huge number of women who have returned to exercise too soon, and too intensively, and have aches and strains on their pelvic floor function," says RenĂ©e Knowles, a physiotherapist specialising in women’s health and clinical Pilates at Up and Running sports medical centre in Dubai. "You must remember that you have just performed one of the most incredible things – developing and giving birth to a tiny human. Time and patience are vital."


Respect for the healing process is key. If you had an uncomplicated, natural delivery, you might feel tender for five weeks or more, and it is advisable to wait until your six-week, post-partum check-up before returning to gentle exercise. MUM-of-two Emma Lea knew she had a problem with her weight when she couldn't carry her young son upstairs.

"Half of my life has been about trying to lose weight,” Henry Roberts said. He was telling me about his decision to have a surgery that would reduce the size of his stomach by seventy-five per cent. Roberts grew up in Queens, the son of an M.T.A. worker who avoided the chemicals in canned foods “before that was fashionable,” he said. Decades ago, he successfully quit smoking and drinking. Managing his weight has been more difficult. 

“You can’t just quit eating altogether,” he said. He had opted for a procedure called a sleeve gastrectomy—the stomach is surgically narrowed to resemble a sleeve—but this was not Roberts’s first attempt at a surgical treatment for obesity. “I can’t remember the exact year, but I know I had the lap-band procedure the weekend that Michael Jackson died,” he told me.

Diet or exercise: What's best for the middle-aged heart?

If you're a middle-aged couch potato in serious need of boosting your heart health, is it better to exercise or diet?

New research says dieting, exercising or a combination of the two can all get the job done about equally well as long as you lose some weight.

But the study authors added that exercising in tandem with dieting is probably the best way to go.


The researchers designed the three study interventions so people participating would drop about 7 percent of their body weight -- through any method -- over a roughly three-month period.

The study showed it didn't seem to matter which intervention people chose to lose weight. Participants in all three groups saw their lifetime cardiovascular risk drop from 46 percent to 36 percent.

"Exercise and a low-calorie healthy diet are both known to improve risk factors for cardiovascular disease, even in the absence of weight loss," said study lead author Edward Weiss. He's an associate professor at Saint Louis University's department of nutrition and dietetics, in Missouri.
